Lower blepharoplasty with fat removal is a surgical procedure in which the prolapsed fat compartments of the lower eyelid are accessed and reduced. The fat can be removed through either the transconjunctival approach, through the inner surface of the eyelid, or the transcutaneous approach, through a lash line incision, depending on whether skin excess is also present.

  • Fine scissors to open each compartment without disrupting adjacent structures
  • Monopolar cautery to coagulate and remove fat safely with haemostasis
  • Toothed forceps to hold fat during removal without excessive traction on surrounding structures

Protecting Adjacent Structures

  • Identification of the inferior oblique muscle, which runs between the medial and central fat compartments and must not be damaged
  • Bipolar diathermy for haemostasis near neurovascular structures
  • Retractors and eyelid plates to protect the globe throughout
